Security Policies
Bring About Changes LLC is subject to the record retention laws of Arizona and ARS 12-2297. Retention of records
A. Unless otherwise required by statute or by federal law, a health care provider shall retain the original or copies of a patient's medical records as follows:
1. If the patient is an adult, for at least six years after the last date the adult patient received medical or health care services from that provider.
2. If the patient is a child, either for at least three years after the child's eighteenth birthday or for at least six years after the last date the child received medical or health care services from that provider, whichever date occurs later.
3. Source data may be maintained separately from the medical record and must be retained for six years from the date of collection of the source data.
B. When a health care provider retires or sells the provider's practice the provider shall take reasonable measures to ensure that the provider's records are retained pursuant to this section.
Clients may request a copy of their chart or that of their minor child at any point during the record retention period. Request must be made in writing to the listed address for Bring About Changes LLC.
Receipt of the request will be acknowledged by Bring About Changes LLC and records will be mailed within 30 days of acknowledgement of the request.
Bring About Changes LLC provides records for free within 6 months of the last visit; after this period, a fee of $20 will be charged and must be paid in advance.
Recording
No party may record conversations or therapy sessions without prior consent from all parties of interest. Recording is at the discretion of the therapist and recording conversations or therapy sessions without the therapist’s consent represents a breach of the therapeutic relationship and services will be terminated.

Compliance with Privacy Regulations
Bring About Changes LLC is in compliance with all applicable federal and provincial/state regulations with respect to the use of technology for healthcare. Bring About Changes uses HIPPA compliant encrypted e-mail, faxing and Electronic Health Records. If sessions are tele-health the platform for meeting is HIPPA compliant for privacy.
